I hereby apply for a license to use the TollTag(s), which, subject to all applicable rules of the Greater New Orleans Expressway Commission and
other applicable governmental entities and agencies, will allow me to gain passage via designated lanes located on the Lake Pontchartrain Causeway.
I agree that I have read, understand, and will be bound by the terms of the Agreement on this Application.
If I have selected one of the credit card options on this Application and listed a card account number, I hereby authorize you to charge periodically
the credit card indicated for the amounts necessary to satisfy my obligations under this Agreement. I understand that the initial total payment due
with submission of this application is the Prepaid Fee above for the toll account. The Low Balance Amount for this account will be the sum of the Low
Balance Amounts for all Tags issued under this account as specified in the Tag Schedule above.
